As a customer and user of our services, you have the right to receive information about the handling of your personal data and to submit requests for the exercise of your rights.
Purposes and legal basis for data processing
We use your personal data exclusively for the purpose of fulfilling your requests for the performance of a specific requested service (primarily the production of laboratory results), so that you can obtain discounts on services related to the total price of the services provided, and exclusively for our own information system in terms of informing you about medical analyzes, as well as to comply with our legal obligations. We use the data only for specified, clear, legitimate and expected purposes and do not forward or use them for unforeseen or unexpected purposes.
The processing is carried out in accordance with the General Data Protection Regulation (“GDPR”) and Croatian laws and regulations.
We do not collect data that is not used to perform the requested service.
The data we collect may include:
– Name and surname / title
– Gender
– Date of birth
– Doctor information
– Notes relevant to determining the findings (e.g. information on previous illnesses, therapies, data on hormonal status or other relevant data that affect the result)
– Requested medical services
– Email or fax address
– Postal address
– If you do not pay in cash – information for non-cash payment
Notification via email
At the request of the client, we send the findings – the results of medical analyses by email to the address(es) we receive upon receipt of the request for processing.
For newsletter delivery, as well as for managing recipient lists and unsubscribe records, Poliklinika Ksaver uses the Mailchimp platform and takes all necessary measures to protect data in accordance with the General Data Protection Regulation and Croatian laws and regulations (GDPR). You have the right to accept or refuse to receive our notifications.
Recipients of personal data
The recipients of personal data are the owners of the data themselves, collaborating laboratories in the country and the European Union connected to the Ksaver Dental Clinic organizationally or by contract and who are themselves subject to and obliged to store data according to the Personal Data Protection Regulation (GDPR), and, when requested by the user, the doctors appointed by them.
User rights
We provide you with the following rights:
– Right to access (information about the scope and type) of your data
– First, to correction (applies to data you have provided, but not to the results of medical analyses)
– Right to be forgotten
– Right to object to the use of personal data
Right to access
You have the right to information about your personal data that we manage and store in memory. If you are the legal guardian of a user, you also have the right to information about the personal data of the person for whom you are the legal guardian.
Right to correction
You have the right to have inaccurate personal data concerning you corrected, which we will do within a reasonable time. We do not change validated medical analysis results. Taking into account the purposes of the processing, you have the right to complete incomplete personal data, including by providing an additional statement.
Pravo na brisanje (pravo na zaborav)
Imate pravo na brisanje svojih osobnih podataka, bez nepotrebnog odgađanja, ako ne postoji zakonit razlog za daljnju obradu tih podataka s naše strane (npr. ako podaci više nisu nužni u odnosu na svrhe za koje su obrađivani). Ako takav zakonit razlog ipak postoji, o tome ćemo Vas detaljno informirati u sklopu odgovora na Vaš zahtjev za brisanjem.
Pravo na prigovor oko upotrebe osobnih podataka
Ako se obrada Vaših osobnih podataka zasniva na ostvarenju naših legitimnih interesa, imate pravo u svakom trenutku uložiti prigovor na takvu obradu osobnih podataka u mjeri u kojoj se obrada odnosi na Vaše podatke. Ako uložite takav prigovor:
• vezano obavijesti („newsletter”) – nećemo upotrebljavati Vaše osobne podatke u tu svrhu;
